Use this form to confirm that a participant has completed all three stages of The Window to the Soul Experience™.

Submission of this form does not automatically issue a Certificate of Completion. Lavanda Red will review the completion information and assign the participant's official certificate number after completion has been verified.

Please submit one form for each participant.

All official certificate numbers are issued by Lavanda Red. Facilitators should not create or assign certificate numbers.
